Living with heart failure means watching for small changes every day. This log gives each morning one row: your weight and the change from yesterday, blood pressure, pulse, six common symptoms to mark, and your zone.
The green, yellow and red zone pages set out the signs of a good day, when to call your care team and when to call 911, drawn from MedlinePlus, the American Heart Association, Cleveland Clinic and Mayo Clinic. There is a page for your own care team's numbers, too.

Questions
Good to know
Where do the zones come from?
From MedlinePlus, the American Heart Association, Cleveland Clinic and Mayo Clinic. They are general guidance and do not replace your own care team's action plan.
Can I fill it in on my phone?
Yes. Fill it in on a computer, or on a phone or tablet with the free Adobe Acrobat Reader app. Or print it and use a pen.
Is there a version for outside the US?
Yes. Both US Letter and A4 are included, and the A4 edition says "your local emergency number" instead of 911.
Is this medical advice?
No. It is a log for recording information to share with your care team. Never start, stop or change a medicine because of your readings without talking to your doctor first, unless your care team has given you a written plan. In an emergency, call 911.
Can I get a refund?
Because the files can be copied, all sales are final. If a file will not open or download, or something is missing, email sk@elderlydaily.com and we will fix it or refund you.
