Blood Pressure & Medication Log for Seniors

A blood pressure log only helps if it is easy to keep and easy for the doctor to read. This one gives you twelve months of big boxes for two readings each morning and evening, a tick chart for up to ten medicines, and a summary page at the end of every month.

It also includes the American Heart Association blood pressure categories, what to do if a reading is higher than 180/120, and seven steps for an accurate reading at home.

Questions

Good to know

Why two readings each time?

The log has room for two readings taken one minute apart, morning and evening. Follow whatever routine your own doctor gives you.

Does it tell me my target?

No. Your doctor sets your personal target, and the log has a place to write it. The American Heart Association categories are included for reference.

Can I fill it in on my phone?

Yes. Fill it in on a computer, or on a phone or tablet with the free Adobe Acrobat Reader app. Or print it and use a pen.

Which paper sizes are included?

Both US Letter and A4.

Is this medical advice?

No. It is a log for recording information to share with your doctor. Never start, stop or change a medicine because of your readings without talking to your doctor first. In an emergency, call 911.

Can I get a refund?

Because it is an instant digital download, all sales are final. If a file will not open or download, email sk@elderlydaily.com and we will make it right.
