The ON/OFF diaries cover every half-hour from 6 am to midnight, for three days before each of four visits. Mark whether you were asleep, OFF, ON or ON with dyskinesia, and when each dose was taken, so the neurologist sees how your days really go.
Twelve months of medicine charts give every dose time its own check box. The On Time, Every Time pages, from the Parkinson's Foundation, help make sure medicines are given on time in hospital, with a page for the ER and two cut-out wallet cards.

Questions
Good to know
What if I don't know the terms ON, OFF and dyskinesia?
The diary explains ON, OFF and dyskinesia in plain words, along with dystonia and freezing, before you start marking the half-hour boxes.
Is it affiliated with the Parkinson's Foundation?
No. It is built on guidance from the Parkinson's Foundation and the American Parkinson Disease Association, but ElderlyDaily is not affiliated with or endorsed by either organization.
Can I fill it in on my phone?
Yes. Fill it in on a computer, or on a phone or tablet with the free Adobe Acrobat Reader app. Or print it and use a pen.
Is there a version for outside the US?
Yes. Both US Letter and A4 are included, and the A4 edition says "your local emergency number" instead of 911.
Is this medical advice?
No. It is a diary for recording information to share with your neurologist and care team. Never change, skip or stop a Parkinson's medicine without talking to your neurologist first.
Can I get a refund?
Because the files can be copied, all sales are final. If a file will not open or download, or something is missing, email sk@elderlydaily.com and we will fix it or refund you.
